Prediction of Porto systemic Collaterals using MR Elastography in Non-alcoholic Fatty Liver Disease

Background and Aim: MR Elastography (MRE) is a non-invasive gold standard to assess liver fibrosis in Non-Alcoholic Fatty Liver Disease (NAFLD). Porto-systemic collaterals (PsC) signify presence of clinically significant portal hypertension (CSPH) & associated with complications variceal hemorrhage, ascites overt encephalopathy. FibroScan shows low sensitivity specificity detect CSPH NAFLD, the aim this study evaluated stiffness (LS) threshold level using MRE for detection PsC indicate NAFLD. Method: 392 NAFLD subjects (257 males, 135 females) undergoing between 2015 2022 were retrospectively analyzed. During sequencing, additional T2 weighted images obtained spleen size PsC. The correlation LS, FIB4 index was determined by Spearman Rank test. ROC analysis cut-offs prediction Result: mean age 47 years ± 13 had median LS score 2.7 kPa± 2.8 kPa. 104 (26.5%) advanced (LSM-5 kPa). Presence significantly correlated (r=0.543, p<0.001) (r=0.478, p<0.001). at cut-off 6.15 kPa (sensitivity-76.9%, specificity-77.1%, AUC=0.87) compared (cut-off-1.98, sensitivity-74.4%, specificity-74.3%, AUC=0.81). A regression model MRE, splenomegaly platelet count showed an accuracy 90.2% whilst index, platelets demonstrated 87% predicting form Conclusion: Elastography, kPa, reliable method hence patients alone combining FIB4-index, similar can be beneficial community settings where MRI may unavailable.
